Please bring a list of any current medications you are taking for your doctor to review when you arrive.

Please provide a list of any medical conditions such as artificial joints or heart valves, medication regimens such as aspirin or anti-coagulants, or any serious surgery or injury, etc.

If you have dental insurance, please bring the necessary documentation. Doing so will save you and us time and will enable us to provide treatment in the most efficient manner possible.
